In multivariable regression models controlling for patient characteristics, LEP inpatients who did not receive professional interpretation at admission, or at neither admission nor discharge, had a longer length of stay than patients with an interpreter on both admission and discharge (the reference category). The increase ranged between 0.75 and 1.47 days (Table 4). Patients with no interpreter on either day had an adjusted LOS coefficient of 1.49 days (SE 0.46, P<0.01), and patients with an interpreter at discharge but not admission had a coefficient of 0.76 days (SE 0.32, P=0.02), while having an interpreter at admission but not discharge was not significant (0.25, SE 0.25, P=0.33). Interpretation at admission appears to have the greatest effect on LOS.

Caveats #

Critically, the data do not record how often family members or untrained staff provided informal interpretation, nor whether bilingual providers spoke directly with patients — both unmeasured factors could confound the association between recorded professional interpretation and length of stay or readmission. The authors also note socioeconomic status, mental status, and exact day-of-admission interpreter use were not captured, and the single-site setting limits generalizability.
